Health System Intervention

Goals:

Enhance the quality of diabetes care
Improve diabetes outcomes
Enhance provider-community relationships
Enhance provider-patient relationships

Accomplishments:

Over 25 Physicians and 150 clients have been recruited; recruitment for both continues
Physician assessments and surveys were developed and are being completed.
Two of the three health sites have conducted Cultural Compentency training with their physicians.
Client evaluation forms and processes are under development.
The Health System Intervention work group meets monthly to plan, develop, and oversee the intervention activities.

Future Plans:

Continue recruitment of physicians and clients
Organize community and REACH family activities that include participation of REACH physicians in order to help them learn more about the community and the people they serve.
Develop evaluation plan for Family Health Advocates.
Complete the development of all policies/procedures for FHAs collecting data and empowering clients to better manage diabetes.
