Family Level Intervention
Goals:
- Increase healthy eating and leisure-time physical activity
- Enhance family-health provider relationships
- Increase access to quality health care
Improve clients ability to control diabetes with self-care
- Increase access to community health promotion resources

Accomplishments:
- 7 Family Health
Advocates (FHAs) were trained in patient empowerment techniques,
diabetes self-management, health care consumer skills, computer
set-up, internet search skills, diet and nutrition, and the benefits
of physical activity.
- Approximately 150 clients with diabetes
have been enrolled; 142 have completed in-home baseline health
surveys.
- FHAs have begun group classes on diabetes
with their clients.
- Over 40 clients have received an internet
computer device and one-on-one training on how to access health
information on the internet.
- This REACH Detroit website, with relevant
health links and a database of community resources, was launched.
- FHAs are teaching group classes using
a culturally tailored curriculum to increase healthful eating
and physical activity among their clients.
Future Plans:
- FHAs and health providers will participate
in semi-annual case conferences to promote better quality health
care and understanding.
- FHAs will lead cooking demonstrations
for clients and their families on healthy low sodium recipes and
low fat food selection.
- FHAs will deliver group classes to clients
using a culturally tailored curriculum to increase and improve
diabetes self-management skills.
- FHAs will make home visits to clients
and their families to provide individual follow-up of group classes
on diabetes, dietary habits, and physical activity.
