Community Level Intervention

Goal:

Increase the capacity of the REACH communities to increase diabetes awareness and improve the quality of the neighborhood environment.

Objectives:

  • Identify, document and disseminate information about community social and physical resources that support healthy lifestyles.
  • Increase connections between community members and health information and education resources that support diabetes awareness and healthy lifestyles.
  • Increase community access to healthy foods and recreation opportunities.

Community Level Intervention

Accomplishments:

  • Conducted a Community Asset Survey of community organizations to identify resources that promote healthy eating and exercise; developed Community Asset Information System, available on this website
  • Established working relationships with city and community organizations to plan and develop community resources.
  • Recruited and trained neighborhood resident Community Health Advocates to develop community resources and lead community awareness and social support activities
  • Conducted presentations regarding diabetes, healthy lifestyles and REACH at health fairs, organization and community meetings throughout Eastside and Southwest Detroit
  • Hosted two "Healthy Soul Food" community education activities in Eastside Detroit.
  • Received a "Level One Promoting Active Communities Award" from the Governor's Council on Fitness for Southwest Detroit.

Community Level Intervention

Future Plans:

  • Work with the Detroit Health Department and the Healthy Eating and Exercise to Prevent Diabetes (HEED) project to expand mini-markets that provide fresh produce at wholesale prices in accessible community locations.
  • Host "Healthy Latino Food" community education activities in Southwest Detroit.
  • Continue to work with community recreation facilities to increase safe locations and programs for recreation and physical activity.
  • Develop and disseminate REACH newsletter.
