The Detroit Community Action Plan
In 2000-2001, members
of the REACH Detroit partnership in the Detroit area developed
a Community Action Plan to address the problem of diabetes and related
conditions. The partners spent a year meeting with people in the
community, many with diabetes, and developing the Community Action
Plan.
This detailed plan, with a focus on preventing
diabetes and its complications in African Americans and Hispanics,
is now being implemented. The Community Action Plan consists of four interrelated
